Total Body Irradiation (TBI) is used in the treatment of leukemia. Total marrow irradiation (TMI) is a form of TBI that focuses the radiation to the major bone-marrow sites where the cancer cells reside. Both TBI and TMI can be performed by Volumetric Modulated Arc Therapy (VMAT). VMAT is a high-precision technique in radiotherapy, which makes it possible to reduce the dose in the lung and, if required, in other Organs At Risk (OAR). The aim of this master thesis was a comparison of VMAT-TBI and VMAT-TMI treatment technique. One major part was the analysis and comparison of the technical parameters in the planning of VMAT-TBI and VMAT-TMI with the aim to determine the main differences with regard to parameters such as number of iso-centres, field arrangement or treatment time. The second main part was an evaluation of the planned dose distributions. The quality of the Planning Target Volume (PTV) coverage in VMAT-TBI and VMAT-TMI were compared, and the effect of organ sparing was quantified. In the end, the advantages and disadvantages of VMAT-TBI and VMAT-TMI were discussed. There is not yet a distinct prove that TMI is better than TBI from a technical point of view. What remains for both techniques is a high effort on personal and time.
